  • For starting treatment that includes a controlled substance as part of your treatment plan, we require proof of an Annual Wellness Exam (AWE) within the last year, or one performed with Circle Medical, before starting a treatment plan
  • We do this in order to ensure we have a full picture of the health and wellbeing of our patients
  • We are not able to see or treat patients under the age of 18
  • You may submit any previous ADHD and AWE records prior to your appointment for your provider to review

  • Costs may vary widely based on your particular insurance plan
  • In 2022, more than 75% of Circle Medical patients with insurance paid $35 or less per appointment
  • AWEs are typically 100% covered by most insurance plans
